Then return back to complete the forms under the "sign" section below. (Skip the scheduling step if you have already scheduled.)

Before your first appointment please review and complete the forms below.

You're all set!  To maximize all patients', as well as the doctor's time:

• - Please arrive arrive on time.
• - Please bring ID, insurance card, and payment with you on your first visit.
• - Please wear comfortable/athletic clothing, as you will be active during the exam and treatment.
